Services:
Multi-Community Care & Homes, Inc. serves adult individuals in four CRS (Community Residential) settings in the community of Litchfield. CRS homes provide 24-hour supervision. MCCH also serves individuals who live on their own or with their parents/family, in our In-Home Family Support and Respite Services Programs.
Community Residential Setting (CRS):
MCCH has four Community Residential Setting sites in the community of Litchfield, MN. Each home is licensed to serve 4 individuals with developmental disabilities. Multi-Community Care & Homes is licensed to admit individuals who are eighteen years or older; unless a variance has been granted.
In-Home (IH) Family Support Services:
These services are available to individuals of all ages from children to elderly who remain living in their parents/family home, or their own home. Services are designed and tailored to fit the individual needs of the people we support and to provide needed support to them and/or their family. Families are actively involved on an ongoing basis in the delivery and evaluation of services. MCCH serves individuals with a wide range of abilities and disabilities. For example, Down Syndrome, Cerebral Palsy, Autism and Asperger’s as well as those individuals with more pervasive cognitive developmental disabilities.
Semi-Independent Living Services (SILS):
These services are for individuals who possess the capability to live independently in their own residential setting with supports.
Direct Support Professionals (DSP):
Our dedicated Direct Support Professional (DSP) team promotes MCCH's mission by enhancing the lives of those we support. DSPs are trained and knowledgeable. They attend monthly meetings, do in-service training, conferences, and seminars to enhance their knowledge and keep updated on the best practices for supporting the people who receive services. MCCH emphasizes on developing skills around the needs of the people we support and promoting a positive and safe environment for learning new skills. All programs are supervised.
Assistance:
DSPs assist with health and safety issues, medication administration, money management, nutrition and meal preparation, grooming and hygiene, personal cares, academic skills, physical activities, behavior supports, community integration, and recreation/ leisure activities.
